Approximately 6.7 to 7.6 million adults in the U.S. are currently living with OUD and nearly 90,000 adults are killed by an opioid overdose every year. Despite the prevalence of medication treatment for OUD (MOUD) and overdose prevention modalities, few patients actively seek out care due to a vast number of barriers. To be exact, among the 4% of U.S. adults who needed OUD treatment in 2022 (about 13 million individuals), only 25%, or 3 million, received recommended medications (CDC, 2022). Barriers to treatment can include: affordability, access, lack of readiness, patient and provider stigma, and lack of trust (American Public Journal of Public Health, 2022). These findings underscore a need to increase use of MOUD.
To understand these barriers on a deeper level, it is helpful to look at the Stages of Change Model. Throughout the stages of pre-contemplation, contemplation, preparation, action, and maintenance, patients experience differing barriers and needs. By applying this model in tandem with MOUD, practitioners can successfully lead patients out of ambivalence and into maintenance and eventually permanent change via successful engagement.

- Discuss how the Stages of Change model applies to opioid use disorder (OUD) care.
- Identify barriers to accessing care and Medications for Opioid Use Disorder (MOUD) utilization as well as skills and opportunities to engage with clients for change at each stage.
- Explore practical strategies for creating stage-appropriate client engagement to care and MOUD.
